There’s a lot of attention around solving social determinants of health. However, many of these issues do not follow the health care paradigm and are related to how hard “the system” is to navigate. Erine Gray, the founder and CEO of Aunt Bertha, will share data and insights about how health systems and health plans are partnering with community-based organizations and health plans to collectively address some of these needs.
